Please print and complete all forms as indicated.
If you do not have access to a printer, contact school@riverviewlutheran.org or call the office at 920-733-3728 to arrange packet pickup or mailing with Mrs. Krueger.
ALL RLS STUDENTS
One form per FAMILY
Alternate Household Income Form
(ALL families must complete)
Extracurricular Permission Form (ALL Families with students in Grades 3-8)
Wrap Around Care Survey Form
(ALL families with students in Grades 3K-4K)
Wrap Around and After School Care Policies & Registration Form
(Families using these programs)
Simply Giving Program Form
(Families utilizing a
payment plan -
fill out only the top section)
Handbook Agreement Form
(Families will receive this form along with the Handbook before the
school year begins)
One form per PARENT

RLS Registration Form
that was included in your information letter with boxes completed and information updated as needed
Checkbook or cash to make payments - families utilizing a payment plan will need an additional check to complete the Simply Giving Program Form
